Bringing these completed forms with you will save some time on your first visit. If you are not able to complete them in advance, we will be happy to give you copies to complete when you come to our office.

- Completed intake forms (or arrive early to complete them at the office)
- Photo ID
- Insurance card(s) — electronic versions are accepted
- Physician’s referral or prescription (if applicable)
- Any imaging reports (X-rays, MRI, etc.) related to your condition
